Bleeding with Mirena

I had my 4th child a month and a half ago. I went to the Mikva about 4 weeks later, and at 6 weeks postpartum I got a Mirena IUD. That day I had some significant staining, but based on what I read on your website that seemed to be a valid dam maka (my doctor verified I wasn't bleeding beforehand, it started immediately after, and just lasted about a day).

A week later I started staining again. I've been trying to follow the advice on your website and don't look at stains or toilet paper. However, this seems to be more than staining – more like a period when on birth control – and it's gone on more than a day. It seems unreasonable to keep changing maxi pads with my eyes closed. I realize this may happen quite a bit in the next couple of months. How do I determine when it should be considered niddah?

Unfortunately it is quite common to experience irregular staining or bleeding during the first 3-6 months after having the Mirena inserted. After those initial months many women experience lighter menstrual periods, or even no period at all.

It can be difficult to distinguish between heavy staining and a light flow. If the bleeding is lighter than a period, it is considered staining and you may take precautions against becoming niddah from it by wearing colored underwear or a pantyliner, and waiting 15 seconds after urinating before wiping. Additionally, we recommend abstaining from intercourse until 24 hours after the staining subsides.

However, if the bleeding is heavy enough to be considered a flow, it does render you niddah. If you have to wear a pad (not just a light pantyliner), or if the bleeding is heavy enough to soak through your clothing, you can assume it is a flow and you are niddah.

If you become niddah from the irregular bleeding, you may need to avail yourself of certain leniencies to help you successfully complete the clean days, such as reducing the number of bedikot performed, and changing your white underwear more frequently to prevent staining from accumulating to the size of a gris. If you do experience difficulty completing the clean days, please don't hesitate to get back to us for further guidance.
