Is exchanging or the husband giving the wife a small gift on the night of tevilah something that "everyone" does?

Answer

No, it is not something that everyone does, nor is it an established minhag of any sort.

However, couples may find it meaningful to give or receive gifts on mikveh night or express their affection for one another in ways that will enhance their loving feelings on the night of their physical reunion.  Open communication regarding how each would like to experience their evening together can help align expectations and prevent misunderstandings.
